Records Retention Policy
In compliance with state laws, I maintain client records as follows:
For Clients in Arizona:
Pursuant to Arizona Revised Statutes § 12-2297:
- Adult Clients: Records are retained for at least six years after the last date of service.
- Minor Clients: Records are retained for at least three years after the client’s 18th birthday or for at least six years after the last date of service, whichever is later.
For Clients in California:
According to California Business and Professions Code § 4999.75:
- Adult Clients: Records are retained for a minimum of seven years from the date therapy is terminated.
- Minor Clients: Records are retained for at least seven years after the client reaches 18 years of age.
Access to Records
Clients wishing to access their clinical records must submit a written request prior to the expiration of the retention period. Please note that records for inactive clients beyond the specified retention periods are not maintained.
